    本年度(2022年度)は2023年度から2024年度にデータを収集するための準備作業を行った。 1.研究手続きの詳細を検討し、確定した。研究の実行可能性に鑑み、無作為化割付試験による2群の群間比較研究を、1群のみのオープントライアルに変更し、次なる無作為化割付試験のサンプル数確定のための効果値測定を主たる目的とした。サンプルサイズを20名へと変更した。研究実施機関の選定と、研究参加者の包含基準、除外基準の確定を行った。標準化された評価(CAPS-5(Clinician Administered PTSD Scale-5th version))の講習に研究者が参加した。評価質問紙として、PCL-5、BDI-Ⅱ、DES-Ⅱを用意し、他に、被験者への説明文書、同意書を作成した。唾液検体の保存のための冷凍庫を確保し、検体採取場所(評価実施場所)から冷凍庫までの運搬、冷凍庫での保存方法を確立した。 2.標準化された介入をデザインした。EMDR Instituteが作成したWeekend1 & 2トレーニングマニュアルを元に 、必要かつ十分な介入マニュアルを作成した。この中には、ターゲットを探すための標準的手続き及びツール、安全な場所を創造するための標準的手続き、ターゲットのアセスメントを標準化するためのツール、ターゲットを処理するための標準的手続き、を含めた。 3.研究代表者が所属する研究機関において、申請を行い、倫理審査を受け、承認を得た(2022-44)。

    Whether using bilateral stimulations to positive targets are effective or not is controversy. In experiment 1, all of three conditions: REM (Rapid eye movement), SEM (Slow eye movement), EF (Eye fixation), were assigned to thirty undergraduate and graduate students. All of conditions increased vividness and emotional intensity. Focus attention to external stimulation could be minimized for good result. In experiment 2, all of three conditions: SEM (Slow eye movement), STEC (Slow tactile with eyes closed), IEC (Imagery with eyes closed), were assigned to thirty undergraduate and graduate students. In results, all of conditions increased vividness and emotional intensity. Both results are different from the Holnsveld et al.(2011). Procedural differences such as how to deal with fading imagery could be contributed to.

    In order to investigate the changing pattern of brain functional location and the autonomic nervous system during EMDR, [oxy Hb] in brain blood flow was measured by 24 channel probes of Near Infrared Spectroscopy(NIRS) in four subjects. Skin conductance, skin temperature, plethysmogram, and respiration volume were also measured. By comparing between pre and post EMDR, clinical indices had significantly decreased. [oxy Hb] of orbitofrontal cortex(OFC) showed rapid increase with recalling traumatic memory but soon began to decrease. In order to investigate the effect of horizontal eye movement on safe place imagery, experiments using university students were conducted. The sense of safety or calmness were decreased with horizontal eye movement and the depth of experiencing imagery was increased with eye fixation.

    Comparing perceptual/sensitive and conceptual recalling as method of recalling positive imagery, more decrease in negative feeling and more increase of positive feeling were observed in perceptual/sensitive recalling than conceptual one. And improvement observed in depressive group was superior to that of non-depressive group. Recalling positive memory or imagery can ameliorate negative feeling as cognitive reconstructuring can. But, effect of adding eye movement to recalling was not clear in depressive undergraduate students. Adding eye movement to recalling positive and negative memory can decrease heart rate, while significant correlation with improvement of feeling was observed in only negative memory in normal undergraduate students.

    In the first study, continuous attention to external stimuli was investigated in three groups from local area volunteers : PTSD group (n=10) who shows PTSD symptoms and has traumatic memories, high stress group (n=8) who shows PTSD symptoms, but does not have traumatic memories and normal group (n=9). (1)PTSD group scored less than high stress group and normal group in digit span task. High stress group did not differ from normal group. Difference could be attributed to traumatic experience. (2)In the index of subjective attention to current stimuli, normal is better than high stress, and high stress is better than PTSD group. The traumatic experience is not only factor causing the difference. In the second study, multiple regression analysis was applied to inventory data including attention, self-preoccupation, PTSD symptoms, and depressive symptoms. Inventory was collected from five hundred sixty four normal adults. Hyperactive symptoms explained attention, and attention explained depressive symptoms. Reexperience (intrusion) explained depressive symptoms. In the third study, indices of continuous attention were measured before and after EMDR treatment of ten clients who has PTSD symptoms. Digit span task score was improved by EMDR treatment. Subjective attention to current stimuli was improved by EMDR treatment. EMDR could improve their controllability of attention. In the forth study, a single case of PTSD was investigated in PTSD symptoms, heart rate change during session and attention. In early period of treatment, hyperactivity and intrusion was improved and counting score was improved and heart rate decreased during recollecting traumatic memories. In the later period of treatment, score of avoidance was relatively high and focused, relationship between symptoms and heart rate was not observed.

    Six hundred twenty-five undergraduate students were distributed an inventory which includes traumatic past events, IES-R (Impact of Event Scale- Revised), and whether they wanted to receive therapy or not. Four hundred fifty-three (male: 181, female: 264, unknown: 8; average age=20.2(SD=1.77)) completed all questions. They were divided them into two groups: thirty-three people who reported serious traumatic events like sexual abuse, death of important persons, traffic accidents, and burying were called the traumatic stress group, and four hundred twenty people who reported mild stressful events were called the normal group. Analyzed by t-test, traumatic group was significantly higher than the normal in total and avoidance subscale of IES-R. When analyzed a hundred eighty-four people whose total score was higher than 25(suggested cut-off point), the traumatic stress group was significantly higher than the normal group in avoidance subscale. Two clients who were sexually traumatized and had symptoms of complex PTSD, were treated by EMDR (eye movement desensitization and reprocessing) and measured external ear canal temperature and heart rate during therapy sessions. A twenty-one-year-old undergraduate student whose IES-R score was around 45, was disciplined violently by her mother, and sexually abused by brother. In forty-seven sessions, including thirty EMDR sessions, many traumatic memories were reprocessed and many symptoms were alleviated. And, (1) Right ear canal temperature was higher than left (25-39 ℃), (2) Both temperatures were higher at the end of EMDR than at the beginning, (3) Left ear canal temperature tend to increase as the number of sessions increased, (4) Validity of cognition scale correlated with external ear canal temperature and heart rate. A thirty five-year-old single mother, whose IES-R score was 43, complained of separation anxiety with her child, headaches, and insomnia due to the Kobe earthquake. Although she had a strong fear of the therapy, in the seventeenth session, three rapes were disclosed. Rape memories and memories in which her stepfather beat her mother when she was five years old were attempted to be reprocessed in eleven EMDR sessions. However, she did not show improvement and dropped out after 39 sessions. (1) Right ear canal temperature was higher than left by.25 ℃, (2) The heart rate decreased by three beats from before to after EMDR, (3) In complete sessions the heart rate was lower than incomplete sessions by 10.58-12.25 beats.

    ゴキブリに対して強い恐怖感を持ち、そのきっかけとなる記憶が明確で、さらに治療を希望する女子大学生21人を無作為に統制群、逆制止群、EMDRの3群に振り分けた。実験の第1日目には3群にそれぞれの介入を約30分行い、その前後に様々な指標を測定した。介入は、統制群はマンガを読むなどして休憩してもらい、逆制止群は筋肉を弛緩させて、恐怖イメージを浮かべさせた。EMDR群は恐怖イメージに焦点を当てながら眼球運動を行った。さらに、1週間後の第2日目にフォローアップとして第1日目と同様の指標の測定を行った。測定した指標は、主観的な指標として、(1)その記憶の映像の鮮明さ、(2)映像に対する否定的な感情の強さ(SUD)、(3)望ましい認知を信じられる程度(voC)、(4)ゴキブリに関連した様々な課題をどのくらいできそうかというセルフエフィカシ-、(5)ゴキブリ入りのゴキブリ捕獲器を捨てる課題直前の状態不安(STAI-S)の上昇、生理的な指標としては、(6)ゴキブリ入りのゴキブリ捕獲器を捨てる際の心拍数の上昇、行動的な指標としては(7)ゴキブリ入りの捕獲器を捨てる際の様子であった。結果の分析はまだ継続中だが、これまでの分散分析では、(3)のvoCにおいて、EMDR群の1週間後の上昇が統制群より傾向として大きかった。1週間後に測定した(5)状態不安(STAI-S)の課題直前の上昇が、EMDR群において他の2群より傾向として小さかった。今後、セルフエフィカシ-、心拍数、行動評定のデータを分析する予定である。これまでのところ、ゴキブリ恐怖の低減に関してEMDR群が統制群や逆制止群より優れた効果を持つようである。1週間後のフォローアップの後からは、3群ともにEMDRによる治癒的介入を十分な改善が見られるまで最高3セッション継続し、大幅な改善をどの指標においても示した。今後、長期的な効果の維持を確認するために、半年後にフォローアップを行う予定である。
